Funding supports Newborn Connections post partum, in home services for monolingual, Spanish-speaking mothers in eastern Contra Costa County, who would not be seen by other in home social service agancies. Child Health & Disability Prevention Program (CHDP) (back to top)
Information:
Free health exams for infants, children and teens (u to age 21). Home visits provided by Public Health Nurses and Community Health Workers. The Department of Health Services has developed an electronic "gateway" at CHDP provider offices and clinics to link children with temporary full scope Medic-Cal coverage. The Gateway offers preventive care for children, care for children when they get sick, pre-enrollments in comprehensive health insurance, and a doctor that knows your child (a medical home). They also work with families to obtain regular preventive health assessments for infants, children, and adolescents, Coordinate follow-up of identified health problems, and refer those with suspected health problems for necessary diagnosis and treatment. Help children find medical and dental homes for their health care.

The prenatal care guidance program serves women who are Medic-Cal eligible regardless of immigration status. Does case management until child is 15 months, all counseling, does in home visits while mother is pregnant. They generally help with any assistance for pregnant moms and new parents.
